familywalk2Many young girls and grown women play with their hair. Often they unconsciously twirl it around a finger, especially when they’re bored or tired. It’s a harmless habit. Of more consequence is trichotillomania, a disorder characterized by a recurrent, irresistible urge to pull hair out of the scalp, eyebrows, eyelashes and other parts of the body, resulting in noticeable bald patches. According to the National Center for Biotechnology Information, trichotillomania may affect as much as 4% of the population; women are four times as likely as men to be affected.* “Trichotillomania is not just a nervous habit,” says clinical psychologist Dr. Francine Rosenberg of Morris Psychological Group, “although it may be triggered or made worse by stress. In some people the disorder is mild and manageable but for many the compulsion to pull out the hair is overwhelming and if not treated can have serious consequences for health and well-being.”

Understanding Trichotillomania

Trichotillomania is generally classified as an impulse control disorder, meaning it is driven by an uncontrollable impulse or urge to perform a harmful act. The individual feels increasing tension that can only be relieved by performing the act; afterward, there is often a sense of shame or regret. Depression and anxiety are common among those with trichotillomania, as are other body-focused repetitive disorders such as skin picking or nail biting. “People who pull their hair are not trying to hurt themselves,” says Dr. Rosenberg. “They have a neurologically-driven need to pull and although doing so acts as an immediate soothing mechanism, repeated episodes of hair-pulling and the disfigurement that results cause significant distress.”

It isn’t known exactly what causes trichotillomania but there may be a genetic component to the disorder; having a close relative who is affected increases an individual’s risk. An imbalance in the brain chemicals serotonin and dopamine might play a role. It usually develops between the ages of 11 and 13 and while it is also seen in younger children – even as young as a year old – those cases are often mild and resolve after a year or so without treatment. For those for whom it is a lifelong disorder, complications include abrasions and infections in the area where the hair is pulled and digestive problems for those who eat the hair. “Emotional distress is a significant issue for most people,” says Dr. Rosenberg. “They try to style the hair to cover the bald patches or wear hats or wigs or false eyelashes to disguise the condition. Embarrassment and shame may cause them to shy away from social and job opportunities, leading to isolation, low self-esteem and depression.”

Treating Trichotillomania

While medication such as selective serotonin reuptake inhibitors (SSRIs) may reduce some symptoms of trichotillomania, the most effective treatment is a combination of education, medication and psychotherapy. The most successful results are typicall from cognitive behavioral therapy (CBT), an approach in which patients learn to change their thoughts, feelings and behaviors so they can recognize and manage the impulses that lead to pulling without giving in to them. With CBT, therapists will use a behavioral technique known as Habit Reversal Training, a three step procedure designed to increase awareness, to teach relaxation strategies and to help identify a behavior that is incompatible with the hair pulling. This behavior, known as a “competing response” often involves using the same muscles as in the hair pulling but used in a discreet manner that would not easily be noticed if done in public. For example, clenching one’s fist and pressing their arm to their side are common competing responses for hair pulling.

“Trichotillomania can be a stubborn, chronic disorder,” says Dr. Rosenberg. For many people, it is a way of dealing with negative emotions such as anxiety, loneliness or stress, but the humiliation and embarrassment that result exacerbate those emotions. Early detection and intervention are important since, with the exception of very young children, the disorder seldom goes away without treatment. Psychotherapy can help manage the emotions and eliminate the impulsive behavior that wreaks havoc with health and quality of life.”

It’s Never Over – Survivor’s Guilt

heartshinyIt’s never over. That’s the inescapable truth, whether we accept it or not. Once your life had been threatened from the inside, you may learn to live with your new reality and even find a way to put it on the back burner. But that brush with death will inevitably spring to the forefront of your mind, prompted by anything that makes you think “it” could be happening again.

There are examples of close calls everywhere you look: the young woman who almost died of an aneurism while she was hospitalized having a baby (a case where location probably saved her life), the guy who was “cured” of thyroid cancer 34 years ago, but never misses his yearly test. The event that shapes the futures of survivors may have happened many years ago, like summer camp. Only unlike camp, this experience is terrifying. Even when you push it to the back of your mind and think it’s over, it’s never over.

I have survivor’s guilt, and I’ve got it bad. It seems that every time I turn around, someone I know who has faced mortality and received a reprieve is back in the trenches, fighting for his or her life. Only this time, probably because there was a first time, the stricken one isn’t as young or as strong as before. The fight is harder. The likelihood of winning is smaller.

It makes me crazy that people I know aren’t as healthy as I am.

And yet I’m long past my expiration date, and probably healthier now than ever before. With hardly any effort on my part. Without making those life changes that so many survivors initiate, when they finally get a chance to refigure their future with their health in the forefront.

This is what’s the same for all of us: we don’t know if “it” will happen again. We can’t ever be sure. We can’t store that period when we were being treated in the back of our minds and forget about it. It’s the other shoe that hangs over our lives, and we can never completely stop waiting for it to fall. Even if we think we’re fine – are we? The medical world throws around the word “cured” as though it’s real, but that’s a dangerous lie. Accepting it as truth could allow us to take our eye off the ball. There is no cure for cancer. And who can say if that artery wall will stand up to a normal life span? We can’t know with any degree of certainty. What happened to us took us to the outskirts of normal, from which there’s really no way back. Personally, that’s why I never take those great deals on subscriptions to magazines. One year is about all I’ll ever be able to be sure of.

I do this because taking action seems to mitigate dwelling too much on the worst possible possibility. I think that’s what meant by enriching your life by having it threatened.

womanarmupToday, a lot of women are suffering from baldness and thinness of hair. In many cases, it is a medical problem. But, sometimes baldness and hair loss can occur from genetics, hereditary, some form of damage, poor diet, lack of circulation, etc. As far as genetics, a parent can have fine or thin hair and as one ages, baldness occurs. The same growth pattern can be seen as their children get older.

Many women love experimenting with their hair. They like to color, obtain curls through permanent waves, and today, a lot of women are chemically straightening their hair. All of these procedures will allow for beautiful results, but, if done too often, they will cause damage. The damage comes from over-processing, which will result in hair loss, dry and brittle hair, and sometimes, scalp damage.

Women are busy working, transporting kids to practices and other functions, taking care of home and the family, and other duties. They miss meals or pick up something in the midst of multi-tasking. Stopping for fast foods or eating something quick, but, not necessarily nutritious. The hair and scalp are nourished by the foods eaten, causing the hair to appear healthy and alive. When the hair looks limp and dead, either it is over-processed or the person has a very poor diet.

In order for hair growth to be stimulated, the blood needs to circulate properly. Therefore, the scalp needs to be rejuvenated by massaging it and the hair needs to be brushed. Try to massage the scalp at least once a week and the hair should be brushed daily. To obtain healthy hair and prevent hair loss, make a few minor changes and the results should be seen fairly quickly.

In order for hair growth to be stimulated, the blood needs to circulate properly. Therefore, the scalp needs to be rejuvenated by massaging it and the hair needs to be brushed.

If a woman is undergoing chemotherapy or radiation, she will see some sort of hair loss or even total baldness. There are solutions available until the hair grows back. But, the patient will need to inquire about the coverage, in regards to her insurance plan. Many women opt for the prosthesis because it looks and feels very natural. The prosthesis can be removed daily or it can be applied to last for months at a time. The color and hair texture will more than likely match almost perfectly. They are very popular among hair loss patients.

Lastly, the impact hair loss has on a woman is detrimental. Many women suffer in silence, too embarrassed to tell anyone or allow themselves to be seen without a scarf, wig, or extensions.
